Lost, Stolen or Damaged Passports
If your passport has been lost or stolen, attempt to locate it by retracing your recent path and checking with the police station closest to the place where you lost the document. If you are unable to find it, you should submit an application for a replacement as soon as possible. Please obtain the items listed below before requesting a replacement.
The Embassy recommends that you obtain a police report for a lost or stolen passport, to document the event and protect your personal identity. Be aware that interpreters are not readily available at all police stations in Czech Republic. A police station is located near the Embassy at Vlasska 362/3, and the phone number is 974.851.730.
Additionally, the Foreigner’s Police station, which usually has English speakers, is located at Olsanska 2, Prague 3; the phone number is 974.820.238.
If additional personal documents have been lost or stolen, please see ‘Preventing Identity Theft’ for suggested security measures.
If you discover your passport is lost or stolen after business hours, please gather the above documents and return to the Embassy on the next business day during ACS office hours. Please note that the Embassy cannot replace passports after hours or on weekends.
Passports that have been damaged or are in poor condition should be replaced, even if the expiration date has not passed. For security reasons, it is important that the document be readable and the photo is intact.
Method of Application
Each passport applicant must apply in person at the ACS office. An appointment is not necessary if you have urgent travel within the next 10 working days and you must apply during ACS office hours.
