I forgot to hand in my departure record (FORM I-94) when I departed the United States, what should I do with it?

If you have a white card (I-94) in your passport when you left U.S., please mail it with your name, date of birth, passport number, and date of departure to

U.S. Customs and Border Protection Air Sea Passenger Operations,
Room 5.4D,
1300 Pennsylvania Ave., N. W.,
Washington D. C. 20229.

If you don’t do it you will be identified as an “overstay”!!!
