How to Schedule an Interview

Please use the process below to schedule your non-immigrant visa interview.

Step 1: Go to the NIV Appointment System page, go to “Choose a help language” and select “cesky”. Hover the cursor above the English to see it written in Czech.

Step 2: At the bottom of the page click the button named “Electronic Visa Application Forms”.

Step 3:  At the bottom of the page click the link called “DS-156: Nonimmigrant Visa Application”.

Step 4: Fill out EVAF. The form questions are in English, but applicants can refer to this instruction sheet in Czech for detailed instructions to prepare the form. Print all 3 pages, including the barcode.  It is very important that the barcode be printed clearly.

Step 5: When you have answered all of the questions click “Continue” at the bottom of the screen.  Then print the DS-156, including Page 3, “Supplement to Nonimmigrant Visa Application” which includes barcode number.  

Step 6:  Return to the NIV Appointment System page and click the “Schedule Appointment” button at the bottom of the screen.

Step 7: The “Barcode number” on the screen should match the number from Page 3 of your EVAF.  (If your barcode number does not appear, you should type it into the text field provided on the screen.)  Click “Submit” to view the appointment calendar, and select a date and time for your interview.  Select an appointment time, enter your personal information and click “Submit” to schedule your appointment.

Step 8: Print out your receipt listing the date and time of your interview.

Step 9: Follow these instructions to pay for your visa interview and passport delivery by courier. 
IMPORTANT: You ONLY need to pay the following fees at the bank before your visa appointment:  (1) the U.S. Embassy, for your visa interview and (2) All Ride s.r.o., for delivery of your passport.  You DO NOT need to pay any other company or agency for any other service if you arrange your appointment by yourself.

Step 10:  On the day of your interview, the consul will decide whether you will receive a visa in accordance with U.S. law. If you qualify for a visa, you must leave your passport at the Embassy. It will be sent to you with your visa via a courier service in a few days. If a visa is not granted, you will get your passport back immediately and you can call courier service 222 518 818 and ask for a refund of the courier service payment.
