Non-Immigrant Visas
Interview Checklist
BEFORE YOU BOOK AN APPOINTMENT PLEASE MAKE SURE THAT YOU HAVE ALL THE NECESSARY DOCUMENTS TO BRING TO YOUR INTERVIEW.
1. Valid Passport
If your passport is damaged, or you do not have a page for a visa, please get a new passport before your interview.
2. Original CSOB receipt for interview fees
You ONLY need to pay the following fees at the bank before your visa appointment: (1) the U.S. Embassy, for your visa interview and (2) All Ride s.r.o., for delivery of your passport. Complete instructions are available on the Fees page. Present the original confirmation of payment at the interview and note that this receipt will not be returned to you. Confirmation of payments made over the internet or via telephone will not be accepted.
3. Completed Electronic Visa Application Form (EVAF)
This electronic form is available at the NIV Appointment System page. Please bring all three pages, including page 3 with the barcode number.
4. Photograph
One color or black and white photograph, less than 6 months old, against a light colored background, looking directly at the camera. The photograph must be 2 by 2 inches (5 by 5 cm). Attach this to page 1 of your printed Electronic Visa Application Form.
5. ENVELOPE
A self-addressed medium envelope for your passport. You will not receive your visa at the interview. It will be sent by the AllRide courier service in 3-4 business days. The Embassy offers NO PICK-UP SERVICE .
6. COMPLETED DS-157 Supplemental Nonimmigrant Visa Application for:
- all men between ages 16 and 45 regardless of nationality;
- all visa applicants (men and women) from Cuba, Iran, Libya, North Korea, Sudan and Syria, who are 16 and over;
In addition to the documents listed above, please bring the documents required for these visa types:
Tourist or Business Travelers (B Visa)
Applications for minor children
Student/Exchange Visitors (J, F, or M Visa)
Temporary Worker (H, L Visa) or Athlete or Artist (O, P Visa)
Ship or Airline workers (C1/D Visa)
Tourist or Business Travelers (B Visa)
EMPLOYMENT LETTER: Proof of employment with dates of employment and salary, proof of study, or other commitments that will demonstrate that your stay in the U.S. will be temporary. For business trips, provide a description of your business plans while in the U.S. and anticipated period of stay. You may be required to provide a job contract or pay statements.
ENTREPRENEURS/SELF-EMPLOYED: An entrepreneur (OSVČ) must be prepared to show the business license, a business register statement, tax records, or other financial documents.
PROOF OF FINANCE: Bank statements from the past 3 months and other proof that you can support yourself during your entire stay in the U.S.
Minor children
Applicants under the age of 14 do not need to be present for visa interviews and may be represented by one or both parents. If only one parent is present, that parent should bring a signed letter from the other parent, declaring consent for the child to apply for a visa and to travel to the US.
Student or Exchange Visitors (J, F, or M Visa)
COMPLETED DS-158 APPLICATION FORM.
SEVIS FEE RECEIPT. Most J, F, and M applicants must now pay a SEVIS fee. Please refer to www.fmjfee.com for details and instructions on payment. Please bring an electronic receipt or I-901 receipt to your interview.
Marriage and Birth Certificates. If your spouse and/or children will be traveling with you, please bring a marriage certificate and birth certificates for your dependents.
PROOF OF FINANCE: Proof that you can support yourself during your entire stay in the U.S. This may include scholarship letters, recent bank statements from sponsors, and tuition receipts from the school. The I-20 by itself is not proof that you have the means to pay for your studies.
COMPLETED DS-2019 APPLICATION FORM. J-1 visitors must have a completed original Form DS-2019 that matches exactly the name in your passport;
COMPLETED I-20 A-B APPLICATION FORM. Form I-20A-B (for F1 students) or Form I-20M-N (for M-1 students) properly completed and signed by both the Designated School Official (DSO) and the applicant;
Temporary Worker (H, L Visa), Athlete or Artist (O, P Visa)
I-797 NOTICE OF ACTION: Original I-797, Notice of Action, and copy of the petition your employer filed with U.S. Citizenship and Immigration Services (USCIS).
BLANKET L-1 PETITION FEE (BLANKET L VISAS ONLY): The U.S. $500 fee must be paid to the Consular Cashier at the Embassy after the interview by cash or credit card.
Marriage and Birth Certificates. If your Spouse and/or children will be traveling with you, please bring a marriage certificate and birth certificates for your dependents.
Ship or Airline workers (C1/D Visa)
LETTER FROM EMPLOYER: A letter from your employer confirming your employment and your position.
CREDENTIALS: This may include a seaman’s book, crew identity card, or similar credentials.
Member of the Media? (I Visa)
LETTER FROM EMPLOYER: A letter from your employer stating occupation; whether your position is permanent or temporary and your purpose of travel and length of stay;
PRESS ID: Your credentials issued by a professional journalistic association;
CONTRACT OF EMPLOYMENT: Freelance media workers must also submit a valid contract of employment.
