Non-Immigrant Visas
Beginning June 1, 2007, all nonimmigrant visa applicants must prepare the Electronic Visa Application Form DS-156 to schedule an appointment. Please follow the instructions on the How to Schedule page to prepare your EVAF and schedule your appointment.
Note: the expiration date in the top right corner is not relevant. All the forms linked here can be used for application.

Note: The DS-157 is required for all male applicants aged 16-45 inclusive. You can prepare the form using the Adobe Acrobat Reader software and print it out.
