Application Instructions (Renew Passport)

Please follow the instructions below if all of the following apply:

  • You are under age 16.
  • You are able to present your most recent passport with the application. 

Application Requirements:

  • Complete sections 1 through 22 on the application, using block letters and black ink.  All fields are mandatory.
  • Item 5 - Social Security Number:  You must provide this information, as applicants who fail to provide a valid number are subject to a $500 fine by the IRS.  If you have never had a social security number, please enter zeros in the field.  If you have a social security number but cannot remember it, please call the ACS office.
  • Item 13 – Mailing Address: Complete this item with a Czech Republic address that you use for work, home or temporary residence.
  • Item 14 – Permanent Address: Complete this item with the address you consider to be your permanent location.
  • Item 23 – Oath and Signature: Please do NOT sign the DS-11 form until you are in the presence of the Consular Officer at the Embassy.  You must read the “Acts or Conditions” on page 4 of the application before proceeding.  If any mentioned acts or conditions have been performed by or apply to you, cross out only the applicable portion(s) and submit a signed statement explaining the circumstances.
  • Photo of the child.  One 5x5 centimeter (2x2 inch) color photo is required, displaying your full face with a light background, no head cover and no dark glasses.
  • Child must be present. All children under the age of 16 are required to appear at the Embassy for all passport services. 
  • Child’s most recent passport.
  • Parental consent is required for security reasons.  Both parents must authorize all passport services for children under age 16.
  • Proof of each parent’s identity. A photo ID, such as your current passport, driver’s license or Czech identification, is required.
  • Proof of parentage.  The child’s original birth certificate or similar document must be presented, listing both parents.
  • If one parent is absent, the applying parent must present the following:
  • A signed and notarized copy of the absent parent’s passport. OR
  • A document identifying his/her authority to apply alone
  • Proof of outstanding family circumstances.
