U.S. Police Records

Criminal records and history requests must be obtained from the FBI by forwarding the following information directly to the following:

FBI CJIS Division – Record Request
1000 Custer Hollow Road
Clarksburg, West Virginia 26306

  1. Written request for personal criminal history. The statement must be signed and include your complete return mailing address.
  2. Fingerprints placed on Form FD-258, the standard fingerprint card, which is available online.  Ink fingerprints used for identification purposes can be obtained at the following Prague location:

  3. USD $18 fee, payable by money order, certified check, or credit card to the United States Treasury.

Under certain circumstances, a sworn statement affirming that you have no criminal history is accepted by the Czech Government.  Please confirm this requirement with the appropriate Czech Government office related to your needs. If appropriate, you may request a bilingual statement from the ACS office which then should be notarized by a Czech notary. There is a fee of $ 30.00 for this notarial from the American Embassy Prague, and you must visit the ACS section during office hours.
