U.S. Civil Records

The U.S. Embassy Prague is not authorized to obtain civil records for citizens and cannot provide translations.  Often, it is necessary to obtain documents such as birth, death or marriage certificates from the Office of Vital Records or Department of Health in the city or county in which they were produced. The appropriate office can be located online via the Centers for Disease Control and Prevention or by calling the ACS section during office hours.
